How exactly do I get it to work fresh from the download?
Will Milkshape 3D work with it?

Best is to use Blender or 3dsmax. There are exporters for both tools to convert to Crystal Space maps. You can use Milkshape 3D but then you will have to convert to blender first to export to CS.
